In order for you to perform safely as an athlete, the athletic department requests knowledge of certain information including a complete physical assessment, medical history, and insurance information. All athletes at Adelphi are required to have a completed the above before participating in tryouts, practices, or athletic events.

1. You will be required to have an athletic physical examination completed by your private physician. This physical must include all immunization dates!

2. This completed physical form must be on file with the Adelphi University Health Services. Without this you will not be able to get a physical by Adelphi's doctors. Therefore, check that your physical and immunizations are on file by calling 516-877-6000.

3. IT IS YOUR RESPONSIBILITY TO CALL TO MAKE AN APPOINTMENT FOR YOUR ASSESSMENT. The phone number is 516-248-7444 and is located at 233 7th Street, Suite 101, Garden City, NY 11530. Please make appointments in advance.

** If you are an international or out-of-state student, it is best to schedule your appointment in advance of when you will be on campus for the Fall semester. If you wait until you arrive to make an appointment, it may delay the start of your season**

**We encourage that assessments be done over the summer in order to accommodate all student athletes. Please make your appointment ASAP to ensure an complete assessment before your team starts their season **

Please make sure to download the two forms, health record questionnaire and insurance information, listed below. They must be both be filled out and signed by all freshman, transfer, and returning athletes. The forms can be printed and returned by either mail, fax or in person.
